Chemotherapy at Home in Thane

The nurse's checklist on the day
Identity, drug, dose, dilution, route, rate and expiry - checked twice, once at collection and once at the bedside, against the oncologist's chart. Baseline vitals are recorded before anything is given. Pre-medications follow the written sequence. Once the cytotoxic drug starts, the nurse charts vitals at intervals and keeps the emergency kit open and within arm's reach until the observation period ends.
Between cycles
- Day-3 and day-7 vitals visit where the protocol expects a count nadir
- Blood sample collection at home when the oncologist wants a mid-cycle CBC
- Mouth care and hydration routine taught to the family
- Written escalation route agreed with the family before the first home cycle
