What Must Be In Place First
Before a ventilator is commissioned at home we require: a supervising intensivist, ICU-trained nurses rostered for continuous cover, an inverter or UPS sized for the ventilator and concentrator, a filled backup oxygen cylinder, a self-inflating bag at the bedside, spare circuits, and a written escalation plan naming the receiving hospital. This is not bureaucracy - each of these has prevented a death in cases we have run.
