Getting Compliance Right
Most BiPAP failures at home are not medical - they are a badly fitting mask, an over-tightened strap, or air leak that dries the mouth and wakes the patient. We size the mask with the patient lying in their actual sleeping position, use a humidifier to reduce dryness, and check the nose bridge for redness in the first week. If tolerance is still poor after a fair trial, we escalate to the pulmonologist rather than let the machine sit unused in a corner.
